Extended Health Benefits

This private plan will be part of your benefit package at work.

Check with your Benefit or Human Resources Department for details of your coverage.  Some plans require a physician’s referral for you to be reimbursed for the cost of your treatment.

 

Motor Vehicle Accident Insurance

If you were involved in a motor vehicle accident, you will have coverage through your car insurance.  Your insurance company will require a completed Accident Benefit Package from you and must approve a Treatment Plan submitted by your Physiotherapist.

Please note that under current legislation, if you have an Extended Health Plan (EHP) which covers physiotherapy, your EHP becomes your primary insurer until it’s payment has been exhausted.  All physiotherapy expenses must be submitted to your EHP before submitting to your auto insurance provider.  Any difference between the amount billed and the amount your plan covers should be paid by your auto insurer.

 

Worker’s Safety Insurance Board (WSIB)

How do I proceed with my WSIB rehabilitation claim?

1. Inform your employer of your workplace injury. WSIB is responsible for making health and rehabilitation services available to you.  Once you have a health claim initiated with WSIB, you should be given a claim number and name of adjudicator.

2. Call us to book an initial assessment.  In this session your Registered Physiotherapist will examine your workplace related injuries, discuss with you a treatment plan, begin therapy, and collect all the necessary information to fill out and submit a WSIB rehabilitation insurance claim on your behalf.

Payment Options

An invoice will be rendered at the end of your assessment or treatment and payment is required at that time.  Unless prior arrangements have been made, the patient is responsible for payment.
