You have been sent this letter because we have received a referral in your name for physiotherapy. This letter is to inform you of our office policies.
Coverage/Billing
We are limited in the amount of OHIP sessions we can provide each year. As a result, we have a waiting list for individuals treated with this form of funding. Individuals are also triaged and wait times for treatment can vary depending on the problem. If you have been referred for OHIP and you have extended healthcare coverage, please consider using this instead. This enables people who do not have coverage or any other available means to access physiotherapy. Alternatively, you can pay for your treatment out-of-pocket and avoid long wait times.
If you do have extended healthcare, please bring the information with you and we will try our best to electronic bill it for you.
No Show Policy or Late Cancelation Policy
If you wish to cancel your appointment, we ask that you give us a minimum of 24 hours notice to enable us to fill your spot. If your appointment is on Monday, you are required to phone on the Friday before. However, emergencies do happen, and a late cancellation can not always be avoided. We understand and honour this.
If you do not attend your scheduled appointment, you will be charged a $50 No Show Fee which is payable before your next treatment. This applies to any funding source (ie. Out-of-pocket or insurance coverage including WSIB, OHIP or a Motor Vehicle Accident).
Clothing/Footwear
Please ensure you bring a clean pair of indoor shoes or slippers depending on what you are coming for and appropriate attire to change into such as shorts or a tank top shirt that is easily accessible.
Location/Parking
We are located at 17 York River Drive in the lower level of the Bancroft Business Centre. You are able to drive up to the front door for pick up and drop off, but we ask that you do not block the door for other patients.
There are designated parking spaces for patients with permits closer to the door.
Medication
If you are taking medication, we ask that bring a current list to your first appointment.
Covid-19 or other illness
If you are ill, please cancel your appointment. Our current mask policy is that you have the option to wear or not wear a mask upon entering the clinic. If you are not ill but have any symptoms, please wear a mask. Our staff will do the same. Should you require that your treatment provider wear a mask, we are happy to wear one at your request.
We continue to maintain a high level of vigilance to ensure the health and wellbeing of our patients and staff and ask that you do your part as well.
